Between Appointments (For People with Diabetes)

AFTER YOUR APPOINTMENT:

  • Make plans to follow the healthcare professionals' recommendations. Discuss plans with your Care Partner.

  • Make appointments to have tests done or see a specialist if you need to.

  • Review notes from the appointment, or a printed visit summary, to make sure you haven’t forgotten any key things discussed.

CALL YOUR MEDICAL TEAM IF:

  • You do not understand or have trouble following medical instructions after you get home

  • Test results haven’t been received within a week of the test date - do not assume that no news is good news

  • You don't understand test results

  • You experience any side effects or problems with your medicines

  • Your symptoms get worse (or do not get better)

  • You receive any new prescriptions or start taking any over-the-counter medicines

  • You have any questions about your diabetes self-care routine
