Between Appointments (For Care Partners)

People who talk with or call their healthcare team tend to be happier with their care and have better medical results. 

If the person with diabetes thinks that you are the best person to call, that is OK. If you call, be sure to write down what the healthcare professional says and share the information as soon as possible.

AFTER YOUR PARTNER'S APPOINTMENT:

  • Help and encourage them  to make plans for following their healthcare professional's recommendations

  • Talk to them about making appointments to have tests done or to see specialists (if needed)

CONTACT YOUR PARTNER'S HEALTHCARE PROFESSIONAL IF THEY:

  • Have problems following or do not understand the healthcare professional's instructions

  • Do not receive test results within a week of the test date - do not assume that no news is good news

  • Do not understand test results

  • Experience any side effects or other problems with their medicines

  • Have symptoms that get worse (or do not get better)

  • Receive any new prescriptions or start taking any over-the-counter medicines

  • Have any questions about their diabetes self-care routine
